Owner Agreement *

I have read and understand the following: All dogs must be current on Rabies, Distemper/Parvo booster (or equivalent), Canine Influenza and Bordetella vaccines. Vaccines must be completed at least 2 weeks before staying at our facility. All dogs must be free of communicable conditions or any condition that may jeopardize other guests. While we require all guests to be current on these vaccines, when many dogs are brought together, there is always a risk of communicable disease transmission (vaccines are not fool proof nor do they cover all things that could be transmitted between dogs). Dog’s Day Out (DDO) is not responsible for any costs incurred due to communicable illness. Even though we require all guests to be on parasite preventative, when dogs are around other dogs, there is a chance they will pick up fleas, ticks, or other parasites. Dog’s Day Out (DDO) is not responsible for any costs incurred due to exposure to parasites. Dogs with open wounds, unhealed surgical incisions, stitches, staples, surgical glue, or any other wound, sore, or non-intact skin will not be able to participate in our services. Dogs older than 1 year must be spayed or neutered to participate in our boarding or daycare services. No dogs may come and participate in any DDO services if they are experiencing a heat cycle. Unless otherwise specified by the owner, dogs may be assessed for and participate in group play if DDO deems personality and behavior appropriate for such. Not all dogs are appropriate for group play. DDO reserves the right to determine if a dog is allowed to participate in group play. This may change with each visit depending on attendance on any particular day. DDO will determine group size and makeup based on our guidelines. Dogs may be segregated at any time as behavior or need for rest warrants. While our staff is trained to follow precautions to insure play time is safe and fun, accidents and injuries can sometimes happen. Scrapes, scratches and wounds may occur as dogs play with their paws and mouths when they romp around. DDO is not responsible for any costs incurred due to scrapes, scratches or wounds resulting from group play. Dog(s) may get wet or dirty while playing and will only get a bath if requested (by owner) at normal bathing price. All dogs must be non-aggressive to participate in our services. We reserve the right to declare a dog’s behavior unfit for our environment at any time. I agree to the following: I am solely responsible for any harm caused by my dog(s) while my dog(s) is attending DDO for any service. Any problems which develop with my dog(s) will be treated as deemed appropriate by staff and volunteers of DDO. I assume full financial responsibility for any and all expenses involved. My dog is current on parasite preventative including flea, tick, and intestinal parasites. To my knowledge, my dog is free of communicable disease or any other condition that might jeopardize other dogs. Veterinary Release I give permission to Dog’s Day Out to contact my veterinarian (or any available veterinarian on call) regarding vaccination status or any other health issue regarding my dog(s). I assume full financial responsibility for any costs involved with veterinarian care deemed necessary by veterinarian and/or Dog’s Day Out. DDO will charge $50/trip to take a dog to a veterinarian appointment during normal business hours and $25/trip for medication/food/supply pick up, an additional $25 will apply to any evening or weekend vet or supply trip. If a dog passes away while in the custody of Dog’s Day Out, DDO will contact the owner, as well as the dog’s veterinarian. The dog will be transported to your veterinarian. If a necropsy is requested it will be preformed, at the owners expense, at Breathitt Veterinary Center in Hopkinsville, KY. Payment Policy Payment is expected at the time of service. All services and payments are non-refundable. Cancellation/Pick-up Policy While we understand an occasional emergency or unforeseen change in plans, failure to give adequate notice of reservation cancellations or changes does not allow us to offer your pup’s spot to another guest. Our reservation schedule is very tight, and we often have a waiting list of pups hoping to get in. We depend on our clients to help us make it work for everyone. We work diligently to satisfy as many requests as we can on a daily basis. We appreciate your help and consideration as we want to keep costs and scheduling rules to a minimum. We require 7 days notice for cancellations or changes in reservations or appointments. This includes altering your drop off day or picking up early. You will be charged for the entire reservation originally booked if changes are made without proper notification. Boarding check out time is 1:00pm. Dogs picked up after 1:00pm will be charged a $30 fee per suite. Like a hotel, we need time to clean rooms between guests and must insure we maintain a safe number of guests at all times.
